Attach revenue stamps and submit it at the counter when you receive the result. It is not used for online applications received on or after October 1, 2026 (paid at a convenience store or by bank).

Copy the number on the application receipt (申請受付票) you received at the counter, for example 阪永C25222. For online applications received by September 30, 2026, use the receipt number in the confirmation email. You can leave it blank if you are not sure.

Name of payer

Write the applicant's name as shown in the passport. This field asks for your name written out (記名), not a signature. Leave it blank if you want to write it by hand. If the applicant is under 18, write the name of the person applying on their behalf (申請代理人) (Osaka Regional Immigration Services Bureau guide).
